Enrollments and Withdrawals
Enrollment forms and Withdrawal forms must be completed by the legal parent or guardian in person, at the Main Office (or in the Registrar's Office). The Main Office is open weekdays (except for holidays) from 7:45 AM - 4:30 PM. The Registrar's Office is open weekdays (except for holidays) from 8:30 AM - 3:30 PM. 8:30 AM - 9:00 AM is by APPOINTMENT ONLY.
Enrollment
A custodial parent or legal guardian must enroll the student in person (a government issued ID will be required).
If your student is coming from a Hawai'i Public School, please bring with you:
Release packet from releasing school
Certificate of Release
Copy of health record (Form 14)
Pupil clearance or transfer form, as applicable
Current Report Card or withdrawal/transfer grades
Official Transcript
Copy of SpEd current IEP or 504 records, as applicable
2 Proofs of residence address
If renting: Rental/Lease Agreement and 1 current utility bill
Must have parent/guardian's name
Must have physical address
If own: 2 current utility bills, property tax or escrow documents
Must have parent/guardian's name
Must have physical address
If your student is coming from a Hawai'i Private School, please bring with you:

Please complete the MVA questionnaire (MV1, Rev. 3/2023)
The Home Language Survey (HLS) is required for all newly enrolled students (i.e. new to HIDOE). The Language List may be used to assist with completing the HLS.

Withdrawal
A custodial parent or legal guardian must withdraw the student in person (a government issued ID will be required). This may be done either at the Main Office or Registrar's Office. Student issued laptops and accompanying accessories must be returned by the student's last day of attendance.
